For this assignment use my current job setting: I work as Case Manger in Managed Long term care, and job is to assess elderly patient with chronic conditions to keep them safe in the community.I come across many falls in elderlies on daily basis. I provide education on disease process, falls are number one priority, I discuss Fall preventions, make referrals like CHHA, Physical Therapy, Also collaborate with health providers and help pt getting canes, walker, Personal response system etc.
