You will design a short customer satisfaction survey to be given to physicians at the hospital. The survey will be designed to assess physician satisfaction with a specific area of your selection in the hospital (Some ideas might be: Laboratory results, Chart completion, Pharmacy availability of medications etc).
You will be graded on the following criteria:
Well-designed satisfaction survey (1 point)
Instructions for survey completion (1 point)
At least 7 questions but no more than 10. Questions should be well-worded and relate directly to the customer’s satisfaction (2 points)
Some questions should gather information about the physician or the process (2 points)
At least 2 questions must use a Likert scale (2 points)
At least 1 question must be qualitative (which is a question that asks for the customer’s opinion and has space for them to write in an answer) (2 points)
