The EBP topic for the Capstone project change proposal is the prevention and management of diabetes. My clinical site is a Clinic and the clinical team is educating them about the disease, its prevention, and its management. Patients learn how to monitor their sugar, appropriate diet, exercise, lifestyle changes, the need od med, and keep follow-up appointments.
